Adherence to Long-term Therapies
By: World Health Organization
Published by: World Health Organization

Adherence to therapies is a primary determinant of treatment success. Poor adherence attenuates optimum clinical benefits and therefore reduces the overall effectiveness of health systems. ''Medicines will not work if you do not take them''. Medicines will not be effective if patients do not follow prescribed treatment, yet in developed countries only 50% of patients who suffer from chronic diseases adhere to treatment recommendations. In developing countries, when taken together with poor access to health care, lack of appropriate diagnosis and limited access to medicines, poor adherence is threatening to render futile any effort to tackle chronic conditions, such as diabetes, depression and HIV/AIDS. This report is based on an exhaustive review of the published literature on the definitions, measurements, epidemiology, economics and interventions applied to nine chronic conditions and their risk factors. These are asthma, cancer (palliative care), depression, diabetes, epilepsy, HIV/AIDS, hypertension, tobacco smoking and tuberculosis. Intended for clinical practitioners, health managers and policy-makers, this report provides a concise summary of the consequences of poor adherence for health and economics. It also discusses the options available for improving adherence, and demonstrates the potential impact on desired health outcomes and health care budgets. It is hoped that this report will lead to new thinking on policy development and action on adherence to long-term therapies. more...

Advances in Contemporary General Practice Nursing
By: Patricia Davidson (ed.); Elizabeth Halcomb (ed.); Elizabeth Patterson (ed.)
Published by: eContent Management, Pty Ltd

Escalating rates of chronic conditions, health inequities and workforce shortages challenge health care systems globally (World Health Organization [WHO] 2003). Nurses work in both autonomous and collaborative care practice settings (International Council of Nursing nd). Across the life span and a range of conditions nurses intervene to improve health outcomes for individuals, families and communities. Since the World Health Organization's Declaration of Alma-Ata in 1978 (WHO 1978), health care systems have looked to the primary care setting for driving health care reforms. Primary care refers to health care, based on the best available evidence and culturally appropriate methods, which is readily accessible to people in their communities (WHO 2003). Promoting community development; health promotion; illness prevention; and care of people who are unwell are key initiatives of nurses internationally and are increasing in importance. In developed societies, the general practice setting is a focus of primary care nursing interventions. In general practice, nurses play a critical role in working with health professionals and community health workers to improve the health outcomes of individuals, families and communities. Advances in General Practice Nursing provides a thought provoking comment on nurses working in an evolving sphere of nursing practice. Although nurses have worked in the primary care setting for many years, the role and scope of practice is not as well described as other areas of nursing practice, primarily due to the limited research and scholarship compared with their acute and critical care colleagues. In Meeting a Primary Care Challenge in the United States: Chronic Illness Care, Saxe, a nurse practitioner, describes how advance practice nursing can improve outcomes for chronic illness in the community. Somchit Hanucharurnkul provides an insight into the nurse practitioner model in Thailand and how this model is culturally acceptable and ap more...

Advances in Contemporary Transcultural Nursing
By: Daly, John (ed.); Jackson, Debra (ed.)
Published by: eContent Management, Pty Ltd

Nurses, locally and globally, are facing new and complex challenges on account of living in a culturally diverse and dynamic world. This special issue of Contemporary Nurse (the first Australian-based nursing journal to carry a special feature dedicated to transcultural nursing) provides a timely opportunity to reflect on some of these challenges and on the various ways in which nurses might respond effectively to them. This special issue also provides a timely reminder of the importance and moral imperatives of the theory and practice of transcultural nursing and its emphasis on care as a universal, but diverse culturally constructed phenomenon, that lies at – and must remain at – the heart of nursing research, education, and practice. more...
